State it is part of the Better Access Mental Health Care Plan
Nominate the number of sessions covered with the referral. Medicare allows up to 6 sessions per referral, totalling 10 sessions per year (6+4=10)
State if you are referring an additional 10 covid sessions, bringing total sessions to 20 (6+4+10=20) Covid sessions can only be referred once the initial 10 sessions have been completed.
Include Client's name, Medicare number, Address and Date of Birth
